Services

Community Living & Supports is available to support successful living with family and/or other natural supports in a private home.  It is designed to support the consumer in community activities with family and/or friends and is individualized based on the consumer’s needs.  Consumers will strive to move toward less dependence on other paid supports by learning new skills and retaining skills to maintain health.  Consumers receive a combination of habilitation and personal care services with this service.  This service allows flexibility to adapt to the consumer’s changing schedule and will be naturally occurring to match the consumer’s typical day.

Respite care is a service that provides periodic relief for the consumer’s family or primary caregiver.

Community Networking services provide individualized day activities that support the consumer’s definition of a meaningful day in an integrated community setting, with persons who are not disabled.  Services are designed to promote maximum participation in the community life while developing natural supports within integrated settings.  These services enable the consumer the consumer to increase or maintain their capacity for independence and develop social roles valued by non-disabled members of the community.

Residential Supports for AFL’s consist of an integrated array of individually designed training, activities, assistance and supervision.  The intent of this service is to meet the habilitation and personal care needs of individuals living in Alternative Family Living homes.  It is designed to provide flexibility and reflect the natural flow of a person’s day.
